A Power of Attorney and Appointment of Guardian is a legal document whereby a parent appoints someone (the attorney or guardian) to care for his/her child(ren) during the parent’s absence. This document authorizes the guardian to take and have the care and custody of the child and grants all of the rights, powers, duties and privileges which would normally pertain to a person officially appointed guardian by a Court in Canada. This includes being responsible for the education and welfare of the child and authorizing medical treatment and surgical procedures on behalf of the parents for the child. When a CF member contemplates going on Temporary Duty or leaving for personal reason without the child, a Power of Attorney and Appointment of Guardian is required in this situation. It is also required when a child accompanies a third party in another country. In this latter case, a Letter of Consent should be obtained as well. The selection of the guardian must be done with care and it is strongly suggested that the parent provides as much information as possible to the guardian regarding the health, welfare and education of the child, so proper decisions can be made if the need arises.

This document can be revoked by you at anytime. A revocation form will be provided when you visit our office for signature of the document.
